About InHome Care SA

InHome Care SA (IHCSA) is a South Australian organisation delivering high-quality care and support to older people, with a strong commitment to enabling individuals to remain living safely and independently in their own homes.

Due to continued growth, we are seeking an experienced Community Registered Nurse to join our highly skilled and supportive clinical team.

About the Role

This is a rewarding community-based nursing role offering a high level of clinical autonomy, continuity of care, and the opportunity to build meaningful relationships with clients.

Based from our Mile End office with some flexible working arrangements, you will deliver holistic, person-centred clinical care to clients in their homes, supporting improved health outcomes and quality of life.

Working collaboratively with Care Partners, Allied Health professionals and Home Support Workers, you will contribute to coordinated and high-quality care aligned with the Support at Home and reablement model.

Key responsibilities include:

  • Conduct comprehensive clinical nursing assessments and scheduled client reviews

  • Develop, implement and evaluate individualised care plans in partnership with clients and their families

  • Deliver evidence-based nursing interventions, including:

    • Medication management and clinical oversight

    • Wound care and complex clinical management

    • Catheter and continence care

  • Monitor and respond to changes in client condition, escalating clinical risks appropriately

  • Provide clinical guidance, education and support to clients, families and support staff

  • Provide mentoring and competency-based training to Home Support Workers

  • Collaborate within a multidisciplinary team to ensure coordinated and seamless service delivery

  • Maintain accurate and timely clinical documentation in accordance with organisational and legislative requirements

  • Contribute to clinical governance, policy development and continuous improvement initiatives

This role is ideally suited to a Registered Nurse who values relationship-based care, clinical leadership and delivering measurable outcomes in a community setting.

About You!

You are an experienced and motivated Registered Nurse who thrives in a community environment and is committed to delivering safe, high-quality care.

Essential requirements:

  • Current AHPRA registration as a Registered Nurse

  • Minimum 2 years’ experience in community, aged care or a related clinical field

  • Strong clinical assessment, decision-making and problem-solving skills

  • Demonstrated ability to work autonomously while contributing to a multidisciplinary team

  • Excellent written and verbal communication skills, including clinical documentation

  • Strong organisational and time management skills

  • Current unrestricted Australian driver’s licence

  • Genuine commitment to person-centred care and client empowerment

  • Understanding of the ethical and cultural considerations of working within clients’ homes

Desirable experience:

  • Wound management and chronic disease management

  • Palliative or end-of-life care

  • Experience in aged care community programs or reablement models
