We are a successful not for profit aged care provider, offering comprehensive range of services from residential aged care, independent retirement living, home support and home care services.
We currently have a full-time position available for an experienced Community Registered Nurse to support our Community Team and in the Barossa Valley.
The opportunity:
Reporting to the Manager, Community Care, the primary purpose of the community registered nurse is to provide clinical support to the community team and clients, ensuring that care and services are provided in a caring and supportive manner consistent with the Barossa Village values, to provide an excellent standard of care to our clients.
Responsibilities:
Provide a leadership role in the approach to Clinical care within the Community Team.
Work collaboratively with the Community Care Team to liaise with Clients and/or their representative to arrange and undertake clinical/nursing assessments and development of the clinical components of person-centred care plans.
Attend to Client clinical care needs appropriate for generalised community nursing (e.g. dementia wounds care, pressure areas, medications, general hygiene issues, UTIs) where required.
Essential Criteria:
Registered Nurse qualification.
Experience in coordination of Community Care Client care.
Demonstrated understanding of and commitment to support the frail elderly in their own homes for as long as possible, with the support of individualised services focussed on empowering Clients and respecting the contribution of carers, family and advocates.
Current drivers licence essential.
Experience in dementia management.
